<p>People who have kidney conditions, or who suspect of having kidney problems, usually submit themselves to a series of tests, such as urine routine test, blood urine test, renal function examination, and other pertinent assessments to determine the exact status of their organ. The results of such tests are referred to as clinical values. But how many patients really know what each value mean?</p>
<p>To give the common man or patient an overview of the terms used by medical professionals to refer to a certain kidney test result, here is an attempt to explain each value, as much as possible, in layman’s term.</p>
<p><strong>Creatinine</strong>. It is a chemical waste product in the blood that forms with each normal muscle contraction. The more creatinine there is in the blood, the lesser the kidneys’ ability to function properly.  Healthy kidneys can readily eliminate creatinine from the blood.</p>
<p>People who suspect to have certain kidney condition, and even healthy individuals who just want to have a medical check on their kidneys, are usually required to submit to a series of tests called basic or comprehen-sive metabolic panel (BMP or CMP) to assess their major organs. Particu-larly, they would be obliged to submit to a <a href="http://www.medicinenet.com/creatinine_blood_test/article.htm" target="_blank">creatinine</a> blood test and blood urea nitrogen (BUN) test to examine their kidney function.  If and when these tests produce a result that can be considered clinically abnormal, or when they reveal certain signs of disease, such as diabetes that greatly affects the kidneys, the tests may be used further to observe how the kidney malfunction fares against its corresponding treatment.</p>
<p>Adult men with healthy kidneys have about 0.6 to 1.2 milligrams/deciliters (mg/dL) if creatinine, while women have around 0.5 to 1.1 mg/dL of creatinine.  As shown in the data, women have lower creatinine levels than men do.  It is because women have less muscle tissue compared to men.</p>
<p><strong>Glomerular Filtration Rate (GFR)</strong>. This is a test performed by physi-cians or other medical practitioners to determine how the patient’s kid-neys get along. It specifically makes an assessment on the amount of liquid and waste that passes through the filters in the kidneys, called glomeruli, and out into the urine. The measurement is calculated per minute. A normal healthy person has a constant GFR, which value stays almost always the same.</p>
<p>The test is performed by drawing a blood sample from the vein, usually from the inside of the elbow or at the back of the patient’s hand. The sample is then sent to a laboratory to have the creatinine level tested. Normal values should have a result of between 90ml/minute and 110ml/minute.  Otherwise, the patient is likely to have problems with his kidneys.  If the value is below 60ml/minute, it indicates that the patient has chronic renal disease; and if the value drops to 15ml/minute, it means that the patient’s condition is worse, as his kidneys have already stopped functioning.</p>
<p><strong>Hematuria</strong>. This refers to the presence of red blood cells in the urine. It may occur only once or may be recurrent. Whether the blood is readily seen with the naked eye or visible only under a microscope, hematuria is indicative of a serious problem that has caused bleeding in the patient. The bleeding may originate internally, like from the kidneys, the ureters, the prostate gland among men, the bladder, or from the urethra.</p>
<p><strong>Microalbuminuria</strong>.  This refers to the persistent and abnormal excretion of small quantities of albumin in the urine.  If measured, this discharge usually ranges between 30 and 300 milligrams per day. This abnormality has varied meanings to different patients. For those who have no risk factors for cardiovascular condition, this abnormality may only mean mild renal issue without any clinical significance. But for those individuals who have hypertension, diabetes, and hyperlipidemia, such abnormal excretion requires appropriate medical attention as this may be associated with cardiovascular problem.</p>
<p><strong>Proteinuria</strong>. This is one of the many complications associated with diabetes that signifies serious problem in the kidneys. Specifically, it refers to the presence of excessive amount of serum proteins in the urine, which is a strong indication of kidney damage.</p>
<p><strong>Potassium</strong>. It is a very essential mineral needed for the normal function of all cells, nerves, muscles, and the heart. A very high level of potassium in the blood stream may result in the weakening of the person, leaving him with an irregular heartbeat. It can even be life-threatening.  While low potassium level may result in the weakening of the muscles and muscle coordination, cramping, fatigue, confusion, and heart failure.  Healthy kidneys can readily remove excess potassium in the blood stream.  But if they are damaged, they can no longer function the way they are made to, so the level of potassium can build up in the body.</p>
<p><strong>Urea</strong>.  Also known as carbamide, urea is an organic chemical compound and waste product made by the body after metabolizing protein.  Dam-aged kidneys cannot remove urea from the blood stream.</p>
<p>There may be other terms or clinical values resulting from varied kidney tests but the above are the most common, so far.</p>

<h3><strong>Chronic Kidney Disease<br />

<p><strong>Chronic kidney disease</strong>, or <strong>CKD</strong>, is a condition characterized by a gradual, and in most cases, permanent loss or damage to the kidneys, which decreases their ability to keep the human body in good health. When this happens, wastes, toxic substance, and water would build up in the blood making the person sick, feeling unusually tired and lacking in energy, and having a poor appetite. He may even find it difficult to sleep, develop swollen feet and ankles, muscle cramps at night, and may need to urinate often.  In some cases, the affected individual may develop complications like acidosis or too much acidity of body fluids, anemia or low blood count, high blood pressure, weak bones, disorder in his nutritional health, and nerve damage.</p>

<p>Also known as chronic renal disease, <span style="color: #575757;"><a href="http://en.wikipedia.org/wiki/Chronic_kidney_disease" target="_blank"><span style="color: #575757;"><strong>Chronic kidney disease</strong></span></a></span> or <strong>CKD</strong> is usually caused by diabetes and high blood pressure. But experts also found that other factors may harm the kidneys, such as:</p>

<p><strong>Glomerulonephritis</strong>, considered to be the third most common type of kidney disease, is a group of diseases that gradually inflames the filtration system of the kidneys, and which eventually cause it to fail.</p>
<p>&nbsp;</p>
<p><strong>Polycystic kidney disease</strong>, which is a hereditary kind of ailment, is characterized by the growing of several fluid-filled cysts in the kidneys that prevent these vital organs to function properly. This impairment eventually leads to kidney failure. Polycystic kidney disease has two major genetic forms, namely: the autosomal dominant polycystic kidney disease, or the adult polycystic kidney disease, and; the autosomal recessive polycystic kidney disease, or the juvenile polycystic kidney disease.</p>
<p><strong>Blockage in the flow of urine caused by stones, dilated prostate gland among men, hardening of the arteries, lupus, recurring urinary infection, and cancers</strong> also result in kidney damage.</p>

<p><strong>Chronic kidney disease</strong> has now become an escalating health issue, as statistics revealed a steady increase in the number of affected individuals since the previous decade. Reports have it that the incidence may be due to the growing number of people getting diabetes mellitus, hypertension, obesity, and/or aging population.</p>

<p>These reports should not be a cause for alarm, though, since <strong>chronic kidney disease</strong> (<strong>CKD</strong>) can be avoided with proper care.  It only takes a well-disciplined individual, especially those who are prone to it, to stop or refrain from smoking; manage their blood sugar level as well as their blood pressure; keep physically fit by doing regular exercise; maintain healthy diet and appropriate weight; lessen salt intake; limit alcoholic drinks, and; drink plenty of water every day.</p>

<p>Nevertheless, for those who are already diagnosed with <strong>chronic kidney disease</strong> (<strong>CKD</strong>), it is important to consult with their respective doctors and follow prescriptions and advice accordingly to avoid further complications.</p>

<p><strong>Chronic kidney disease</strong> (<strong>CKD</strong>) has no known cure yet, but it is possible to slow down its affliction or to stop its progress provided the afflicted person observes appropriate treatment and lives a healthy lifestyle. Chronic kidney disease (CKD) is divided in to 5 stages according to the measurement of estimated rate of glomerular filtration, or GFR.  It should be noted that kidney function in first stage is normal, but which is minimally lessened in second stage.</p>

<p>Stage 1: At this point, the afflicted person demonstrates slight kidney damage but without showing any symptoms at all; and so, he may not be even aware he has stage 1 <strong>chronic kidney disease </strong>(<span style="color: #000000;"><strong><a href="http://en.wikipedia.org/wiki/Chronic_kidney_disease" target="_blank"><span style="color: #000000;">CKD</span></a></strong></span>). His GFR is at normal or at high level of more than 90 ml/min. Ways by which a person may know he has the disease are through the following manifestations: medical check up for another health conditions, like diabetes or high blood pressure; the urea or creatinine level within the blood is higher than normal; presence of protein or blood in the urine; results of an MRI, ultrasound, CT scan, or X-ray reveal evidence of damage, and; the afflicted individual has a history within the family suffering from <strong>polycystic kidney diseases</strong> or <strong>PKD</strong>.</p>
<p>&nbsp;</p>
<p>It is recommended, therefore, that Stage 1 <strong>chronic kidney disease</strong> (<strong>CKD</strong>) afflicted persons should properly observe a healthy diet and maintain a healthy blood pressure level.</p>
<p>&nbsp;</p>
<p>Stage 2: Just like in the first, Stage 2 <strong>chronic kidney disease</strong> (<strong>CKD</strong>) patients demonstrate mildly reduced kidney function, but still, they do not know they have the disease since no apparent symptom manifests yet.  However, their GFR level drops at 60 to 89 ml/min.</p>
<p>&nbsp;</p>
<p>It is recommended that patients of <strong>chronic kidney disease</strong> (<strong>CKD</strong>) should observe an active lifestyle by keeping a healthy diet, maintaining a healthy blood pressure level, keeping tab of their blood sugar level, submitting for regular medical check up including a serum creatinine test to measure their GFR, following proper medications, exercising regularly, and staying away from smoking.</p>
<p>&nbsp;</p>
<p>Stage 3: At this stage, <strong>chronic kidney disease</strong> (<strong>CKD</strong>) patients would now begin to demonstrate symptoms of the disease, like: fatigue, excess fluid in the body, a frequent urge to urinate, pain in their back, and sleeping problems.  They would also experience a moderately reduced kidney function with their GFR level drops at 30 to 59 ml/min. At this point when their kidneys’ functions start to decline, patients tend to develop a condition called uremia because wastes may now begin to accumulate in the blood. Besides, they become vulnerable to complications of kidney disease, like high blood pressure, anemia, and/or bone disease.  Patients will now be advised to strictly observe a healthy diet.</p>
<p>&nbsp;</p>
<p>Stage 4:  At this stage, patients would now have a severely reduced kidney function where their GFR level drops at 15 to 30 ml/min.  Dialysis or a possible kidney transplant might be required at this stage. In addition to the complications that have developed at Stage 3, <strong>chronic kidney disease</strong> (<strong>CKD</strong>) patients are now likely to acquire heart disease and other cardiovascular disorders when they reach Stage 4.</p>
<p>&nbsp;</p>
<p>Stage 5: Here, <strong>chronic kidney disease</strong> (<strong>CKD</strong>) patients have come to an advanced state or end of renal disease stage (ESRD), where their kidneys lost almost all of its abilities to function that a kidney transplant or a dialysis be the only option for them to survive. Their GFR now has dangerously dropped to less than 15 ml/min, and toxins have already accumulated in the blood resulting to an overall ill condition.</p>

<p>Urine, or urinary, retention is a serious health condition where the afflicted person suffers from difficulty or inability to completely empty his bladder.</p>
<p>Also known as <strong>ischuria</strong>, <strong>urine retention</strong> generally bothers men in their 50s and 60s, and the likelihood of acquiring the disease may even rise as the person advances in age. Men suffering kidney stones and prostate cancer are likewise prone to urine retention. At times, women and other age groups can be at risk of the condition especially when their bladder is dragged down by a drooping lower part of the colon, a condition called rectocele. Women who’ve gone through recurrent <a href="http://www.bladderkidneyinfection.com/124/causes-and-symptoms-of-bladder-infection/" target="_blank">urinary tract infection</a> (UTI) and gynecologic surgery, and those whose bladder drops out of its usual position, a disorder more popularly known as cystocele, will have higher risk of developing urine retention.</p>
<p>Generally, <span style="color: #000000;"><a href="http://en.wikipedia.org/wiki/Urinary_retention" target="_blank"><span style="color: #000000;">urine retention</span></a></span> is classified according to causes, namely: infectious and inflammatory, neurological, obstructive or physical, and pharmacological.</p>
<p>Prominently, the condition is a manifestation of benign prostatic hyperplasia (BPH), or a growth in size of the prostate, which is a usual occurrence among men as they get older; but it can also be due to other factors. <strong>Urine retention</strong> may also occur when there is a disorder with the muscles and nerves that regulate the urinary tract; or, when the problem originates from the urinary tract itself.  In normal functions, the nerves in the urinary tract signals to the brain when the bladder is full, and the nerves of the brain reciprocate by sending a response signal to contract and release the urine. But in the event when either the nerves in the urinary tract or in the brain fail to coordinate with each other, or when the muscles fail to function properly, the afflicted individual will consequently suffer from an inability to pass urine even if he feels his bladder is full. Malfunction of these nerves can be triggered by certain factors, particularly: vaginal childbirth; ailments like diabetes, strokes, multiple sclerosis; injury to the pelvis; heavy metal poisoning, and; infection or injuries affecting the brain and/or the spinal cord.</p>
<p><strong>Urine retention</strong> can either be acute or chronic.  An <strong>acute urinary retention</strong>, or AUR, is an intensely disturbing ordeal marked by an urgent need to urinate, but failure to pass urine; thus, causing so much discomfort and pain on the afflicted person. His lower abdomen also becomes bloated. The pain most often accelerates when he is exposed to cold weather, has consumed too much ethanol, or has taken medication that prevents his bladder to contract.  Because of this, the afflicted individual needs to be rushed to the hospital for prompt action.</p>
<p>Meanwhile, chronic <strong>urine retention</strong> may not be readily life-threatening but it still has the potential to become a serious condition, and should, therefore, be given medical attention. It causes the afflicted person mild discomfort and frequent trips to the restroom as he feels his bladder is continually full. He can pass urine, albeit finding it difficult starting a stream, and the flow is weak. He also experiences dribbling in between trips to the restroom.  Chronic urinary retention is most often due to an obstruction in the bladder opening.</p>
<p>Initial treatment of <strong>urine retention</strong> involves draining the bladder of urine to avoid permanent damage by inserting a catheter into the urethra. But those whose condition is due to enlarge prostate are given appropriate treatment immediately. Long-term treatment is based on its causes.</p>

<p>Most often, when people experience pain in the back, they mistake it for kidney pain. After all, there is always likelihood that pain in the back is related to kidney pain, especially with the occurrence of signs and symptoms. <h3><em>Pyelonephritis</em></h3>
<p>Defined as an infection of the kidney and the upper part of the urinary tract, is caused by a reflux of the flow of urine from the bladder to the urinary tract due to a blockage along the urine’s flow path. The obstruction along the urinary tract may be due to stones, congenital deformities, tumors, and loss of bladder function from nerve condition, among many more factors. Urine obstruction is likely to intensify the person’s tendency to develop pyelonephritis. <p><strong>Kidney stones</strong> come in varied sizes, shapes, and colors.  It can be as tiny as a grain of sand, or it can grow as large as a golf ball.  The smaller sizes may not cause much pain as they can easily go out with urination. But the larger ones may get stuck on their way down to the urinary tract.  Once the stones get embedded between the ureters and the urethra, they obstruct the regular flow of the urine; and this triggers pain. Most often, the pain is felt at the side and back of the body, and may sometimes even extend to the abdomen and groin. Urine usually comes out cloudy and having a tinge of blood.</p>
<p>Generally, kidney stones do not require surgery unless the pain becomes unbearable.  The smaller ones can just pass on their own with urine, and some, like the cystine and uric acid types, can be diluted with oral medication.  But the larger stones have to be broken down using shock wave procedures; and in severe case &#8211; and only when it is necessary – they be removed through surgery.</p>
<p>One of the best and practical ways that healthy people, as well as those having a health history of kidney stones, should do to avoid developing kidney stone is to drink plenty of water everyday so that their kidneys can properly maintain a healthy body fluid balance, and prevent the stones from recurring.</p>

<p>In anatomy, a kidney is either one of a pair of small bean-shaped vital organs located in the dorsal region, or the lower back, of the human body.  Positioned one on each side of the spine &#8211; although the left kidney is set a little higher than its counterpart at the right &#8211;  its sizes are about 4 inches or 10 centimeters long, and about 2 and a half inches or 6.4 centimeters wide.</p>
<p>The kidneys perform many functions, such as: keeping the blood clean by freeing it of wastes and water; balancing the chemicals in the body; dispensing hormones and assisting in the control of blood pressure; production of red blood cells, and; generating vitamin D to keep the bones strong and healthy.</p>

<p><strong><span style="text-decoration: underline;">Keeping the blood clean</span></strong>.  Even as blood circulates through our veins and arteries, it carries with it some undesirable particles that come from what we take in, be it food or drinks and soup.  Instinctively, the kidneys remove the wastes, excess fluid, salt, and other minerals carried by the blood, and leaving just enough quantities for the body’s requirements. The kidneys’ numerous tiny filters called glomeruli, are the ones responsible for sifting out wastes and water from the blood. The cleansed blood, then, returns to the heart again and recirculates through the same process; while the wastes and extra water enter the bladder through the ureters and become urine, which we eventually discharged through urination.</p>
<p><span style="text-decoration: underline;"><strong>Balancing the chemicals in the body</strong></span>. After removing excess fluid from the body, the kidneys now retain what is called the body’s fluid balance.  Based on the body’s total weight, the kidneys maintain a normal fluid balance of about 55 percent for women, and about 60 percent for men.  If we drink plenty of fluids daily, kidneys remove our excess fluid by making us discharge plenty of urine. Fluid may also be discharged through sweat, breath, and feces. This is how healthy kidneys function.</p>
<p>On the other hand, if we do not take in much fluid, the kidneys will retain the fluid in and we discharge only little urine.  In cases where kidneys fail, these organs become unable to control the fluid balance in the body.  If we do not discharge much urine, we develop the tendency of overloading fluid in our body, and may therefore submit to dialysis and restrict our fluid intake to correct the imbalance. Too much fluid inside our body may cause high blood pressure.</p>
<p><strong><span style="text-decoration: underline;">Dispensing hormones and assisting in the control of blood pressure</span></strong>.  Healthy kidneys produce renin, an enzyme that mobilizes the hormonal mechanisms to regulate the body’s fluid and electrolyte balance. It is especially dispensed at the instance when blood pressure starts to fall. Renin then transforms the blood protein into angiotensin, a peptide hormone that causes the blood vessels to narrow. In turn, angiotensin, impels the adrenal gland to dispense aldosterone, another hormone responsible for the re-absorption of sodium, or salt, and water into the blood.</p>
<p>To recapitulate, these hormones work together in controlling the amount of salt and fluid the body keeps and the manner that the blood vessels expand and contract.  Excessive amount of water, or fluid overload, in the body causes blood pressure to go up; while insufficient amount of the same, or dehydration, causes blood pressure to drop. Moreover, when arteries constrict its width, blood pressure soars high.  This is where renin comes in, as it controls the constriction of the arteries.</p>
<p><strong><span style="text-decoration: underline;">Production of red blood cells</span></strong>. <p><strong>Urinary tract infections</strong> (<strong>UTI</strong>) are serious bacterial problems that invade the urinary system and multiply, resulting in an infection. UTI is painful and can be a life-threatening problem if left untreated. Statistics reveal that about 80 to 90 percent of <strong>UTI</strong> are caused by Escherichia coli (E. coli), the bacteria that are normally found in the rectum, from where they usually enter and travel through the urethra to the bladder and there cause an infection.</p>
<p>In addition, statistics show that there are more than 9 million people all over the world who suffer from urinary tract infection every year. Women, particularly those who are sexually active, are 30 percent more likely to contract urinary tract infection (<strong>UTI</strong>) than men and children because of their urethra are much shorter and also closer to anus. And, of the women, the elderly are more susceptible to the infection than the younger ones due to their weakened immune system, which is no longer potent as it was in their younger years. Since the elderly now enters into a stage where the muscles of their bladder have weakened, there is a greater chance that they cannot flush out properly causing some urine to remain there. When the bladder is not fully emptied during urination, small bacteria creeps its way up into the bladder and accumulates over time and this leads to infection.</p>

Many elderly persons, especially those who are caring for themselves, do not get appropriate nutrients during meal times, probably because they are unable to properly prepare food for reasons like decreased mobility, inability to care for themselves, and/or limited access to assistance; and so, they tend to settle on the same diet they have been taking over the years, mistakenly thinking their nutritional needs have not changed. Because of this, they usually do not have enough calories, proteins, vitamins and mineral intake. For example, senior frail women should have at least three cups of milks and also a cup half of fruits, and fats that should not ever exceed 130 of calories a day.
